Instructions for YSA Recipient and Family

There are two related forms that must be completed to process the scholarship.  The YSA Recipient Form has a section at the end where the Parent Authorization and Waiver Form must be uploaded.  If you have any questions please E-Mail the YSA Chair at [email protected].

• Print the appropriate Parent Authorization and Waiver Form (see link at right)
• Read, sign and date form
• Scan completed form and upload to the completed YSA Recipient Form (see second tab for instructions)

YSA Recipient Form Process

• First review the form (see link at right) and gather all of the following things. 
  • ​Signed Parent Authorization and Waiver Form (see first tab for instructions for this form)
  • Contact information for Summer Program you are attending
  • If possible, an Invoice or Receipt for the Summer Program.  If this is not available at this time, please send it to OATAG as soon as possible as this is needed for OATAG to disburse the funds.
    • E-Mail: [email protected] (as an attachment)
    • USPS: OATAG, P.O. Box 1703, Beaverton, OR 97075 (please notify the YSA Chair when you mail)
  • All necessary information for the form
• Be sure you have all of the documents required before starting the form  (see link at right).  The form does not save data in partially completed forms.
